The Hantsport Collaborative Clinic is seeking a Family Physician to take on a well-established collaborative family practice with flexible funding model and practice management options. Opportunities to diversify your practice may also include: Hospitalist, Emergency Room, Care by Design Program in Long-term Care, and OR assist.

The clinic is a longstanding, full scope, across the lifespan family practice with an integrated EMR (MedAccess). The EMR is located on a shared enterprise with three other clinics in the area, offering opportunities for improved care coordination/continuity, inter-referrals, population-based care and quality improvement. The team currently consists of one Family Physician (retiring), one Nurse Practitioner, one Family Practice Nurse, one full-time administrative support and one part-time administrative support staff.

The team is committed and passionate about the community and work together to optimize patient and family care. The Nurse Practitioner has several years of experience in the practice and is extremely knowledgeable about local services and programs.  The practitioners at the clinic currently provide patient-centered care such as home visits as needed for frail elderly and/or palliative patients.

The clinic offers flexible access to care for patients with dedicated same-day/next-day appointments, a weekly Walk-In Clinic (3 hrs/week), and weekend appointments one Saturday/month.

The team is flexible with scheduling of work and vacations, providing support to one another to cover time away from the office including monitoring of labs/DI and follow-up on patient care.

The clinic offers a pleasant, bright office space and has a great relationship and support from the local pharmacy team.

Hantsport is in a central location, offering the benefits of a rural community, but only 40 minutes to Halifax and a short commute to Windsor and/or Wolfville.  It is also supported by a robust local West Hants Network Council which provides opportunities to network and build relationships with local healthcare providers, problem-solve local issues, and opportunities for shared learning and on-going education such as local speakers and coordination of relevant CMEs. There are also supports available from the local Hants Health and Wellness Team, an interdisciplinary, integrated chronic disease prevention and management team (www.healthandwellnessteam.ca).

Additional Information:

The clinic is adjacent to the local pharmacy and within walking distance of a coffee shop, pizzeria, hardware store and grocery store.

 Hantsport and surrounding areas is a nature lover’s dream, with local trails and recreational programs and facilities available such as library, outdoor pool, ball field, and a dog park all in walking distance.

Hantsport has a P-8 school that is also within walking distance of the clinic.

Hantsport is home to the Hantsport Music Festival in July of each year.
